Corewell Health Hospice & Palliative Care MD/DO in Grand Rapids, Michigan
Corewell Health West Medical Group is recruiting for a Board Certified/Board Eligible Hospice & Palliative Care Physician to join their robust, interdisciplinary team providing Hospice and Palliative Care to patients in the greater Grand Rapids area. Our Palliative Care Division maximizes the skills of our robust the consultation throughout the health system in the Grand Rapids area. Looking for a physician with expertise in pain and symptom management with a holistic, team approach with the assistance of Advanced Practice Providers, Social Workers, Pharmacists, Chaplains, RN Case Managers, and other professionals. The Division of Hospice & Palliative care works in collaboration with integrated Acute Service Lines and includes home palliative care, home based primary care services, the Medicare Care Choices program and one of the largest hospice programs in the state

Essential Functions
Assesses and monitors the medical needs of patients/significant others.
Provides quality medical services to patients in a cost effective, appropriate manner and ensures coverage of case load at all times.
Provides input into the development of policies and procedures as they related to medical practice and quality patient care.
Provides oversight of Physician Assistants and Nurse Practitioners and others providing delegated services to patients.
Evaluates patients'/significant others'/staff educational needs and develops and implements appropriate teaching strategies or delegates to appropriate resources.
Documents in a manner consistent with regulatory standards.
Communicates with other members of the health care team in order to ensure continuity of care and coordination of services. Acts as a patient/family advocate.
Participates in ongoing process improvement activities through the development/application of clinical paths, process improvement and other related activities.Actively works with Spectrum Health to meet financial and patient satisfaction goals.
Qualifications
LIC-Physician (MD) - STATE_MI State of Michigan Upon Hire required Or
LIC-Osteopathic Physician (DO) - STATE_MI State of Michigan Upon Hire required
CRT-DEA Registration - STATE_MI State of Michigan Upon Hire required
CRT-Basic Life Support (BLS) - AHA American Heart Association 120 Days required Or
CRT-Basic Life Support (BLS) - ARC American Red Cross 120 Days required
CRT-Adv Cardiovascular Life Support (ACLS) - AHA American Heart Association If required by specialty or stress test performed 120 Days required
